      I agree to allow my child to participate in the event in The Zone and affirm that my child's participation is completely voluntary. I understand that there are risks inherent in the activities my child will engage in at the event in The Zone which may cause serious injury or even death. I agree to hold Oorah and its agents, affiliates, and employees harmless from all damages and costs to the extent permitted by the law. I also understand that, despite safety precautions, Oorah cannot guarantee that my child will not be injured. My child and I are willing to assume these risks. To minimize the risk, I have instructed my child to obey all the rules, regulations and instructions of the Staff at the event.
                
      In case of emergency, accident, illness or other incapacity which occurs while I or my minor child are using the property, facilities, equipment and/or services of The Zone and/or participating in any sports activities, I give my permission for myself/ourselves and/or my child to be treated by a medical professional and admitted to a hospital, if necessary. I understand and agree that I am/are responsible for all medical and emergency expenses incurred on my behalf or that of my child regardless of whether I have authorized such expenses.
                
      I voluntarily agree to release, waive, hold  harmless, discharge, & agree not to sue the person or entity responsible for administering the event in The Zone or its trustees, officers, employees, agents, students, and staff from any and all liability, claims, actions, demands, expenses, attorney’s fees, breach of contract actions, breach of statutory duty, or other duty of care, warranty, strict liability actions, and causes of action whatsoever, that I might now have or may acquire in the future, arising out of or related to any loss, damage, or  injury, including death, that may be sustained by me or my child, or to any property belonging to myself or my child while participating in the event in The Zone.
                
      I agree that the staff at the event in The Zone shall have the right at their discretion to enforce established rules of conduct and/or terminate my/my child’s participation for failure to maintain these standards, or for actions or conduct detrimental to or incompatible with the welfare, comfort, harmony or interest of the group and its program as a whole.  
                 
      Rules – the following forms of behavior are considered a violation of this document and are unacceptable and could result in the immediate dismissal of the minor(s) at the expense of the parent or guardian: 
      Cigarettes or any tobacco products, alcohol, non-prescribed drugs, and weapons MUST NOT be brought to The Zone.
                
      Using language which is offensive, sexist or racist 
      Fighting, bullying or any other forms of aggressive behavior 
      Leaving camp boundaries without permission 
      Behaving in a manner which is potentially dangerous to self and others 
      Behaving in a manner which damages or vandalizes the property of others or The Zone Camps campuses.

Submitting this registration is a REQUEST to attend the trip.
